Memory Program Annual Membership Consultation Services

Geriatricians have expertise in evaluating, diagnosing, and managing memory problems. Patients enrolled in our annual membership program are provided with continued comprehensive care and support to manage their memory disease. 

The Golden Oak Medicine annual memory program membership fee includes four-yearly office visits and as-needed business hour access and response to questions.

We create a personalized memory program for our clients starting with the following

Initial Visit- Includes learning about our client by obtaining a comprehensive medical, family, and social history followed by a physical examination. We also perform memory testing which includes a series of validated memory tests that we have been trained to perform. We discuss further testing such as blood work, brain imaging, and possible memory testing, followed by education. our initial visit generally takes 2-3 hours. 

Visit 2- Our clients return approximately 6-8 weeks after their initial visit to review blood work and/or imaging results, diagnosis, and discussion of a personalized treatment plan. This visit takes approximately 1 hour. 

Visit 3- Our clients return approximately 3-4 months after their second visit to follow up on their treatment plan, learn more about their memory disease. This visit takes approximately 1 hour. 

Visit 4- Our clients return approximately 6-8 months after their initial visit for repeat memory testing, discussion, and education. Repeat memory testing helps us better understand how a person's memory disease is progressing. A better understanding of memory disease progression can ensure our patients have a better quality of life, help us help you determine when more assistance is needed for everyday activities. This visit takes approximately 1-2 hours. 

Routine follow-up visits- Our Clients return to the office approximately every 3-4 months for continued active treatment, memory testing, and education. Caregivers are involved every step of the way! Research has shown that non-pharmacologic support and treatment of memory disorders improve patient and caregiver quality of life, significantly decrease the odds of placement in a nursing facility and significantly increase time to placement in a nursing facility. Our goal is to keep our clients at home and engaged in their life for as long as possible.

  • Learning to be better caregivers.

  • Identifying caregiving obstacles and challenges.

  • Understanding what it means to be a caregiver for a loved one with dementia.

  • Learning how to navigate the care of a loved one through the health care system. 

  • Gaining practical communication skills with loved ones.

  • Overcoming challenging family conflicts. 

  • Helping with transitions as a caregiver as their loved ones disease progresses.
